A. Yes! Provided you authorize them in writing your permission to handle this transaction and provide a copy of your ID. Your signed and dated authorization must include: Your name, the child’s name and date of birth, and a Statement of what you are authorizing, to whom you give this permission, when (period of time-date), and signed and dated by you. Please provide them with the following documents to bring with them when coming to our office. Proof of address, established by a current statement (within 30-days) of any one of the following: Utility bills (Lights, Gas, or Water), Bank statements (checking or saving), Correspondence from a government department Social Service, IRS, Disability, or Employment), car registration, check stub, or property taxes. Progress report/transcript if they are attending a school outside the district or have a sibling (brother or sister) attending a school out-of-the district. Please bring a copy of their last school progress report or transcript or a copy of the sibling’s progress report/transcript from the school of attendance. The transcript or progress report is the required documentation for the transfer/permit requested reasons sibling or continuation. If enrolling a kindergartener or first grader, include their immunization records, birth certificate, recent health examination, and proof of oral health. If you are missing an item, please bring what you have, and the service team will discuss with you the appropriate steps for the missing item(s).
