In order to successfully enroll your child online you will need to make sure you have completed the following steps: show proof that student has been withdrawn from previous school, acquire the most recent report card, make sure that the physical & dental surveys are completed and stamped by the doctors' offices. Completely fill out all of the following fillable forms below before moving on to the online enrollment process in Aeries. You will need to download these fillable forms to your computer and then upload them into Aeries. Please take into consideration that you would need to upload/fill out each of the following forms only if it is applicable to your situation: *For foster youth; please provide the placement letter. Use *Caregiver Affidavit (Only fill out this form if you are not the legal guardian). All other forms are required.

Please read the following information carefully in order to successfully register online. Download and fill out the following forms if they are applicable to your situation. Please note that if your child has attended a school previously you will need to *withdraw your child from previous school and bring the most recent report card. Fill out the health insurance survey, bring your child's immunization records, students who are registering to 7th-12th grade need proof of TDAP/Whopping Cough vaccination. Only fill out the *caregiver affidavit if you are not the legal guardian of the child you are registering. *For foster youths please provide the placement letters. * If your child has been suspended or expelled from school please provide the placement letters.

Please make sure that you have the done the following *withdraw your child from previous school and have the most recent report card. Have their immunization records up to date, for 7th-12th make sure the student has had their TDAP/whopping cough vaccination. *The following forms should only be filled out in a necessary basis, *caregiver affidavit (only if you are not the legal guardian, *for foster youths please provide the placement letters, * suspension and expulsion placement letters if applicable.
